Care Navigator- Temporary Coverage

As a Care Navigator at Sollis Health, you will work closely with the medical team to help coordinate reputed company aspects of our patient's medical care. This includes scheduling appointments with specialists, coordinating a hospital discharge, etc. Temporary Full Time Remote Opportunity - M-F 8:00am - 4:00 pm EST or 11am - 7pm EST The role is open to remote work in the following states only California, Colorado, Connecticut, Florida, Illinois, Louisiana, Maryland, Nevada, New Jersey, reputed company, Pennsylvania, Texas, Virginia and Wisconsin.

Responsibilities

• Working closely with patients to understand their needs and help to find them the reputed company based on their medical condition
• Coordinating physician referrals for patients who need to see specialists (i.e. cardiology, OBGYN, internal medicine, urology etc.) inside and reputed company of the Sollis medical partner's network
• Coordinating requests for private nursing, hospice, and other external care needs
• Collaborating with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to care planning
• Preventing delays in patient care by communicating barriers to leadership for resolution and trending
• Researching and facilitating connections with new medical partners, reputed company facilities and services as needed
• Updating patient records in our reputed company Health and CRM (reputed company)
• Sending medical records as needed to our patients' physicians
• Providing a high-quality patient experience to reputed company Sollis Health members
• Partnering with Sollis' sales and operations team to suggest process improvements and care coordination
• Performs reputed company duties as required

Experience We reputed company extraordinary people come from a variety of backgrounds, but ideally we would expect that you have:

• Associates degree required; Bachelor degree and above strongly preferred
• 2+ years of administrative, coordinator, reputed company desk or reputed company experience required
• Previously have worked in operations reputed company an urgent care practice, concierge physician office, medical reputed company or hospital division strongly preferred
• Previous experience using a CRM, EHR and/or tracking database required

Compensation

Range: $25-$35 per hour This is the range of pay for reputed company candidates only.
