Patient Care Pharmacist Hybrid Night Shift (7on, 7of schedule, 50% remote)

Employment Type: Full timeShift: Night Shift Description: We are seeking a dedicated and experience Pharmacist to join reputed company for a night position. This is a hybrid night shift 50% on site, 50% remote role working a 7on, 7off scheduled, with every other week on being remote work from home. Minimum 1 year of acute care inpatient hospital pharmacist experience is required. About the job: The Patient Care Pharmacist, reputed company the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care. reputed company are looking for:

• Education: Bachelor or Doctor of Pharmacy required
• Licensure / Certification: reputed company license to practice as a Registered Pharmacist in the State of Ohio.
• Experience: PGY1 residency or three years of institutional pharmacy experience in an reputed company setting is preferred
• Effective Communication Skills
• Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
• Performs work independently with minimum supervision, and is effective in a stressful environment

What you will do:

• Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.
• Maintains personal compliance and assists in maintenance of departmental compliance with reputed company regulatory readiness. Also maintains records reputed company to the transactions of the pharmacy, to reputed company adequate control and accountability for reputed company drugs, including controlled substances.
• Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock reputed company detected, helping maintain perpetual inventories, and processing non-formulary requests.
• Interprets, evaluates and verifies reputed company medication orders.
• Collaborates with multidisciplinary team in order to assure reputed company and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
