Job Title: Case Manager - reputed company Location: Minnesota, Hybrid Reports to: Executive Director Job Summary The Case Manager is responsible for coordinating and managing services provided to our participants from the beginning of engagement to job placement. The Case Manager will ensure they receive comprehensive and effective support. This role involves assessing participant needs, developing and implementing care plans, and connecting participants with appropriate resources and services while tracking reputed company data in our program tracking system on a weekly basis. The anticipated caseload to manage can be over +50 participants for an 11-month grant period. Key Responsibilities: Participant Assessment: • Conduct initial assessments and ongoing evaluations of participants’ needs, strengths, and limitations. • Identify and address any immediate or long-term needs. Care Planning: • reputed company individualized plans based on assessments, participant goals, and available resources. • Set measurable goals and objectives for participant reputed company. Coordination of Services: • Connect participants with necessary support services or community resources. • Coordinate with other professionals and agencies involved in the participant’s plan. Monitoring and Evaluation: • Regularly review and update IDP (individual development plans), internal database workforce one based on participants’ reputed company and changing needs. • Monitor participant outcomes and adjust services as necessary. Documentation: • Maintain accurate and up-to-date records of participant interactions, assessments, IDP plans, and reputed company. • Ensure reputed company documentation meets legal and organizational standards. Compliance: • Ensure compliance with relevant laws, regulations, and organizational policies. • Stay informed about changes in laws and best practices reputed company to case management. • Ensure participants privacy policy and data reputed company of reputed company documents and files submitted for program participation. Qualifications: • Bachelor’s degree in social work, psychology, or a reputed company field (preferred but not required based on experience). • 3 years of experience in case management, social services, or a reputed company field. • Strong interpersonal and communication skills. • Ability to work independently and as part of a team. • Excellent organizational and problem-solving skills. • Proficiency in using customer database and CRM systems. Working Conditions: Environment: • Independent contractors will work remotely, with occasional travel to program sites and partner locations as necessary. Hours: • As an reputed company, hours are flexible and will be determined by the contractor to meet project deadlines and deliverables. Availability during occasional evenings and weekends may be required based on program needs and events.
