Student Centered Support and Interventions. a. Provide direct case services to address academic, social, and emotional concerns. b. Coordinate student/family utilization of community resources and facilitate access of applicable services through school social work delivery model. d. Consult with local schools and the Department of Family and Children Services concerning reputed company forms of suspected child abuse and neglect. e. Serve as a liaison between the district and community services agencies.3. Planning and Program Management. a. Assist in the planning, implementation, and evaluation of programs identified to promote academic achievement. b.
