This position will focus on patients with multiple chronic diseases, CHF, COPD. This position will focus on disease education, identifying and mitigating social influencers of health, coordinating needed services and collaborating with the provider offices to ensure integrated care through the reputed company continuum and improved health outcomes of our patients.What you will need:Associates Degree required, BSN preferred.Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting.Certification in Case Management (CCM) preferred.Experience Requirements:5 years of RN experience.Prior electronic medical record (EMR) experience required. Previous project leadership experience required. Possess strong community resource knowledge. General knowledge of insurance plans and clinical resource managementExcellent oral and written communication skills. Excellent customer service skills. Ability to work independently, setting priorities to coordinate care plan reputed company. Proven leadership skills and ability to work effectively in a team environment. Highly organized and detail-oriented, ability to reputed company multiple tasks.
