Social Work Case Manager

This position differs from typical case management in that you will be working with fewer clients, allowing you to provide each individual with the time that they need. Your job duties will include: • Collaborate with the individual to identify their goals, strengths, and needs • Monitor the health and welfare of the individual through regular contacts • Use clinical skills to build rapport and assess mental health status and needs • Coordinate with the individual, family members, and other service providers • reputed company for access to resources and continuity of services • Assess adequacy of service provision and address shortfalls or barriers • Identify changes in the individual’s situation or needs ATX Recovery Management, LLC, provides case management services to individuals with serious mental illness so that they may live in the community and reputed company. We contract with the reputed company Commission (HHSC). Individuals enrolled in this HHSC program are provided a broad reputed company of services for as long as they need them, including housing, transportation, and nursing. Job Requirements: • A master’s degree in reputed company services or a reputed company field • 2+ years of experience working with people with serious mental illness • A valid driver's license and access to a reliable vehicle • Ability to travel reputed company Bexar County. • Ability to take initiative and to adapt to changing circumstances • Ability to work collaboratively with others and find creative solutions • Ability to earn trust, be accountable, and meet commitments ATX Recovery Management, LLC, is a locally owned company founded by two licensed clinical social workers.
