About the position reputed company Integrated Kidney Care (IKC) is seeking a Full-Time RN Case Manager to join reputed company in the East reputed company, GA region. This position is designed for a dedicated Registered Nurse (RN) who will support patients with reputed company health needs, particularly those with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). As an RN Case Manager, you will play a crucial role in helping patients navigate the reputed company system, ensuring they receive the necessary medical, social, emotional, and financial support. Your responsibilities will include conducting medical record reviews, identifying patient needs, and implementing appropriate interventions to enhance patient care. In this hybrid-remote role, you will work from home while also traveling reputed company your assigned geography for in-person visits to dialysis clinics and nephrology practices. The position requires a Monday to Friday schedule, with flexibility to accommodate patient and nephrology partner availability. You will be responsible for quickly building empathetic relationships with patients and their families, coordinating care transitions, managing ongoing needs, and establishing treatment plans in collaboration with the reputed company team. Additionally, you will address social determinants of health, ensuring that reputed company aspects of a patient's well-being are considered in their care plan. Your role will also involve assessing patients to identify any unreported medical conditions or changes that could reputed company to adverse outcomes.
