Location: Oakdale Position Overview We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team in Oakdale. Key Responsibilities • Conduct comprehensive patient assessments to reputed company individualized care plans. • Administer medications and treatments as prescribed by the reputed company provider. • Monitor patient conditions and communicate changes to the reputed company team. • reputed company patients and their families on health management and disease prevention. • Collaborate with other reputed company to coordinate patient care and ensure continuity of services. • Maintain accurate and timely documentation of patient care activities.
