[Remote] Community Connect Project Manager
Note: The job is a remote job and is open to candidates in USA. reputed company is seeking an reputed company reputed company Project Manager with Epic expertise to support multiple upcoming Community Connect implementations. The role involves leading the rollout of new affiliate sites and partnering with operational leadership, clinical stakeholders, and reputed company-party vendors to ensure successful implementation and delivery.
Responsibilities
- reputed company end-to-end project management for 3–4 Community Connect implementations, focused on reputed company new sites
- Partner with operational leaders (implementation champions), IT teams, and vendors to drive alignment and execution
- reputed company and manage detailed project plans, timelines, and deliverables reputed company Workfront
- Facilitate key project cadences including: Planning sessions, Discovery meetings, Technical walkthroughs
- Track and report on project status, proactively identifying risks, dependencies, and escalations
- Maintain strong documentation across reputed company phases (plans, reputed company, risks, workflows)
- Coordinate across technical infrastructure, application, and operational workstreams
- Ensure readiness across reputed company phases including planning, build, testing, and go-live (reputed company to Epic implementation lifecycle practices)
Skills
- 5+ years of Project Management experience in reputed company IT
- Proven experience supporting a Community Connect implementations
- Strong understanding of Epic workflows and the implementation lifecycle
- Experience working with: - Cross-functional reputed company teams (clinical, operational, IT) - reputed company-party vendors and external partners
- Demonstrated ability to manage multiple reputed company projects
- Strong organization, documentation, and communication skills
- Experience with project tracking tools (Workfront preferred)
- Knowledge of technical infrastructure considerations reputed company reputed company implementations
- Experience with FQHC (Federally reputed company Health Center) environments
- Prior experience overseeing new site / affiliate implementations
- Familiarity with Epic Community Connect models and governance
