Corporate Clinical Trainer - Nurse Practitioner - SNF

Corporate Clinical Trainer and Nurse Practitioner (NP) – Physical Medicine and Rehabilitation (Physiatry) in SNFs- Will travel across the US (25-50%) to help lead fellow providers. We provide training for this role. Our established, rapidly growing company is seeking our newest team member; a Corporate Clinical Trainer / NP or PA specializing in physical medicine and rehabilitation services. We have 4 Clinical Trainers and we're adding a 5th. In this role you'll be training our newly hired APPs our systems, processes, and best clinical practices. You'll be paving the way to our success! With a focus on work-life balance, we empower our team members to make a meaningful impact in the lives of those we serve. Position Overview Our company is nearly 200 APPs strong and growing! We provide physical medicine and rehabilitation in SNFs. We provide training and strong clinical/administrative support. The ideal candidate will have a strong clinical background (prefer in rehabilitation and/or post acute care), excellent communication skills, and a passion for helping patients regain their independence and improve their quality of life.

Key Responsibilities

Train newly hired APPs to do the following:

  • Conduct comprehensive assessments of patients in skilled nursing facilities (neuromusculoskeletal and rehabilitation focus)
  • Collaborate with interdisciplinary teams (physical, occupational therapists, etc.) to develop and implement individualized care plans
  • Monitor patient progress and address barriers to rehabilitation
  • Attend daily clinical meetings for patient status/facility operations updates
  • Foster strong relationships with nursing staff, therapists, and administrators
  • Document encounters and care plans promptly using electronic health records (Doctor Now/Point Click Care)
  • Make recommendations to the primary care team based on patient assessments
  • Participate in chronic care monitoring and rehospitalization prevention initiatives
  • Maintain open communication with the regional clinical supervisor and other team members

Qualifications

  • Licensed as a Nurse Practitioner or Physician Assistant (PA) in Washington
  • Excellent interpersonal and communication skills
  • Ability to work independently and collaboratively
  • Commitment to patient-centered care and improving outcomes
  • Strong clinical assessment skills (neuromusculoskeletal health preferred)
