This position is the rare exception; an opportunity to work for a medical clinic that prides itself on delivering compassionate, personalized patient care. The successful candidate will work one-on-one with patients to identify potential health issues and develop proactive solutions customized to each patient’s personal preferences. You will play a crucial role in helping each patient develop a preventative health mind set and raising the patient’s awareness that self-care is the key to maintaining good health and a longer life. DESCRIPTION This is a unique role for an experienced RN or LPN who is interested in remote behavioral health counseling. You will be providing remote physiologic monitoring (RPM) for blood pressure, blood glucose, weight, and pulse oximetry in combination with remote preventive health care services for primary care clinics to help patients make positive changes to improve their health. This is a challenging role because motivating patients to change behaviors requires different strategies that must be tailored to each individual in order to be effective. Successful intervention begins with identifying risk factors and then implementing appropriate interventions based upon the patient's level of awareness of the negative behavior, willingness to change, and ability to address the obstacles to positive change. A significant portion of your time will be spent with patients discussing sensitive topics such as obesity, lack of self care, depression, and problems arising from Social Determinants of Health. Patients may be embarrassed, defensive, or reluctant to enter into honest, productive, conversations. A qualified candidate must have a great deal of empathy and understanding for the patient’s current situation while demonstrating the ability to ask open-ended, patient centered questions that will elicit meaningful responses from the patient. Understanding each issue from the patient’s perspective is essential in order to develop a personalized care plan with meaningful outcomes for the patient. You will also spend time talking to patients about negative behaviors where the patient may lack the motivation to change, such as smoking, a sedentary lifestyle, poor diet, or alcohol misuse. In some cases, the patient may not even recognize a problem exists. Your role is to ask non-threatening, patient-centered questions to begin a dialogue that will help you understand the patient’s level of awareness of the negative impact of these behaviors. Patience is of the essence; if you push too far, you can damage your relationship with the patient and lose the opportunity to affect positive change. Your job is to build a rapport with the patient over time, and do your best to keep the conversation flowing even if the patient is not ready to acknowledge negative behaviors or initiate positive change at this time. The remainder of your time will be spent documenting the services you provide to each patient, and entering the information into our company software platform and/or the patient's electronic health record. Attention to detail is essential. You will receive training on what information must be documented for each type of service. You will also receive training on how to use the company software platform and the electronic health record software. All candidates MUST have a good working knowledge of computers, Word, Excel, Email, and PDF files, as well as good typing skills. KEY RESPONSIBILITIES

  • Be diligent and professional, logging into the company software platform daily to monitor for clinical alerts and reach out to patients who need assistance.
  • Have a positive, can-do attitude with every patient, every day. Your job is to motivate patients to set personal goals and create a climate in which patients will want to do their best.
  • Ask questions when you are not clear on what to do or how to do it.
  • Ask questions to confirm when you think you know what to do and how to do it. Do not assume.
  • Memorize the information you will need to use every day, such as documentation requirements.
  • Work closely with the supervising physician and the clinical contact to make sure the patients who need additional attention from a healthcare provider are seen in a timely manner.
  • Document the start time and end time for each service provided for each patient.
  • Enter all required documentation into the company software and/or electronic health record software.
  • Enter billing and diagnosis codes for the services you provide.
  • Schedule patients for a follow up meeting if all of the required services could not be performed in the time you were allotted.
  • Proactively call patients who need your support to stick to their commitment to a healthy lifestyle.
  • Be your patient’s biggest cheerleader. Celebrate small wins and milestones!

QUALIFICATIONS AND SKILLS

  • Minimum 3 years experience as a LPN or RN.
  • Licensure in the state required or a multistate license through the NLC.
  • Ability to work independently with minimal oversight.
  • A good working knowledge of computers, Microsoft Office, and good typing skills.
  • Ability to develop positive, collaborative, and professional relationships.
  • Exceptional listening skills and empathy with the ability to see issues from the patient’s perspective.
  • Exceptional verbal skills with an ability to ask a series of patient centered, open-ended questions.
  • Highly organized and the ability to multi-task.
  • Professional conduct at all times.
  • A warm, personable, and outgoing personality.

COMPENSATION/BENEFITS

  • This is a 1099 position for independent contractors.
  • Compensation is production-based and paid monthly per service performed.
  • Pay is based on productivity rather than hours worked.
  • You will determine your own hours, coordinating your availability with your patient's needs.
