Senior Social Worker - Remote Hospice Care
Role Overview Uplift Sobriety Counseling Solutions is seeking a dedicated and compassionate Senior Social Worker to join our remote team. This role is focused on providing essential support and counseling services within a hospice center setting, helping clients navigate their emotional and practical needs during challenging times.
Key Responsibilities
- Conduct comprehensive assessments of clients' psychological, social, and emotional needs.
- Develop and implement individualized care plans in collaboration with multidisciplinary teams.
- Provide counseling and support to clients and their families, addressing grief and coping mechanisms.
- Facilitate support groups and educational workshops on topics related to end-of-life care and emotional wellness.
- Coordinate with healthcare providers to ensure holistic care for clients.
- Maintain accurate documentation and records in compliance with regulatory standards.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited institution.
- Current state licensure as a Clinical Social Worker.
- Minimum of 3 years of experience in a clinical or hospice setting.
- Strong interpersonal and communication skills.
- Ability to work independently in a remote environment.
