POSITION PURPOSE

The Care Navigator, who can be any person with medical experience (Medical Assistant, Certified Nursing Assistant, Emergency Medical Technician, etc.) will meet with members in their homes to complete preventative screenings. As a Care Navigator, you will work with and reputed company physician practices to review medical charts with the goal of capturing missing data that will support care gap closures. In the Care Navigator role, you will also communicate with patients regularly to obtain reputed company statuses, review visit plans, provide education, and collaborate with other professional staff. Additionally, you will coordinate appointments and other service needs for a higher reputed company of care gap closure while meeting the outlined quality measurements.

This position requires travel to New Orleans, around Orleans Parrish, and surrounding areas to provide in-home support to our patients.

DUTIES & RESPONSIBILITIES

  • Work collaboratively reputed company a team setting to provide positive patient outcomes.
  • Demonstrate professional, appropriate, and effective communication with patients, families, and other team members to ensure the continuum of care.
  • Maintain patient confidentiality, following HIPPA guidelines and regulations.
  • Will be required to travel to assigned members’ homes for screenings.
  • Adheres to outpatient treatment center policies and procedures to ensure safety and quality care.

  • Collect and submit data to and from EMR.
  • Demonstrate a positive and professional attitude in providing customer service and quality care.

SKILLS & QUALIFICATIONS

  • One (1) year of medical experience.
  • RMA, reputed company, CNA, and/or EMT experience.
  • Must have the willingness to drive with a clean driving record.
  • Reliable transportation.
  • Excellent communication skills, both verbal and written.
  • Requires problem-solving and decision-making.
  • Must be able to work in a self-directed environment.
  • Must have a willingness to drive and a good driving record.
  • Identify and resolve problems in efficient and effective ways.

EDUCATION, LICENSES, & CERTIFICATIONS

  • High School Diploma or GED required.
  • Associate’s degree is preferred.
  • Medical Assistant schooling is strongly preferred.
  • Medical Assistant certification preferred.
  • Certified Nursing Assistant certification preferred.
  • Emergency Medical Technician certification preferred.
  • CPR/BLS certification.
  • Annual TB testing.
  • Unrestricted driver's license.

PHYSICAL DEMANDS

  • Must be able to frequently drive long distances, which may require a two-hour radius of your designated county (travel time is paid and mileage reimbursement is available).

SALARY RANGE DISCLAIMER

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $19.00 to $23.00 on an hourly basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
