Job Summary

The Family Nurse Practitioner will deliver patient care services through a remote technology platform, working collaboratively with a dedicated team of professionals to provide high-quality care to our patients. The successful candidate will be responsible for evaluating patients, providing healthcare education and counseling, and developing treatment plans to ensure the best possible outcomes for our patients. Key Responsibilities

• Evaluate primary care, acute, chronically ill, and transitional care patients, providing healthcare education and counseling, and disposition planning for patients ranging in age from 18 months and above

Essential Qualifications

To be considered for this role, you must have:

• Basic Life Support (BLS) certification
• A minimum of high-speed/broadband internet connectivity with a download speed of at least 25 download and 3 upload speed
• An unrestricted license in two of the states listed and the ability to obtain multi-state/compact privileges and licensure in non-compact states as required by the business
• Effective verbal, written, and electronic communication skills
• Outstanding organizational skills and ability to multi-task

• Ability to work without direct supervision and practice autonomously
